Battery Won't Come back Up!!

Can't figure this out. My lipo ll.1 3s will not charge back to 12.5.  Very gradually this last month each recharge is lower and lower.  My charge goes green and says it is charged but now it won't go above 11.42 Volts?  Each charge it started going down by about .07 V.  Is it still safe?  Is it bad.  Is my charger lying to me?  Thank you for your help!!!

Which charger are you using? If it is one with balance charging mode, check the individual cell voltages. If it is the kind that only charges through the balance taps, get a better charger.
